Receives all referrals for services. The Admissions process includes a
patient/family assessment and the establishment of services for new patients.
Highly skilled interdisciplinary teams create a comprehensive approach that
focuses on pain relief, symptom control, comfort and support for terminally ill
patients. Services are provided throughout
Provides comprehensive pain and symptom management in beautiful home-like
settings conveniently located throughout the Valley. These units offer state-of
the art medical expertise as well as an environment that promotes comfort and
dignity. Families are always welcome.
Hospice of the Valley also provides services in acute care settings, and at
skilled nursing facilities and residential/assisted living facilities.
One-on-one care in patient/family residence when skilled nursing care is
required and the patient desires to remain at home.
Evening/weekend team assures continuity for patients and families. Staff is
available around-the-clock to consult with families and assist during
emergencies.
Field staff members are supported by work teams focused on education and
research, human resources, finance, information services, marketing,
operations, and triage.
